The Ambulatory Care Pharmacist will provide comprehensive medication management and reconciliation, ensuring optimal therapy and safety monitoring across various disease states. This role involves conducting virtual assessments and coordinating with clinicians remotely, delivering patient-centered care while collaborating with healthcare providers.

Requirements & Qualifications

• PharmD degree.
• California licensed pharmacist.
• Eligibility for CA advanced practice pharmacist (2 of 3: board certification, residency, 1,500 hours).
• 3+ years of clinical pharmacist experience.
• Strong communication and organizational skills.
• Proficient in technology and remote collaboration tools.
