Ambulatory Care Pharmacist

The Ambulatory Care Pharmacist will provide comprehensive medication management, ensuring optimal patient outcomes through effective collaboration with physicians and interdisciplinary teams. This role focuses on medication reconciliation, dose optimization, adherence support, and drug-level assessments aligned with evidence-based standards.

Key Responsibilities

• Provide comprehensive medication management and reconciliation.
• Collaborate with physicians, specialists, and interdisciplinary teams to create individualized care plans.
• Monitor therapy safety and effectiveness, identifying optimization and cost-saving opportunities.
• Support quality-improvement and REMS initiatives.
• Assist with complex prior authorizations and improve workflows.
• Educate patients and maintain ongoing clinical development.

Required Skills & Qualifications

• PharmD degree and licensed pharmacist in CA.
• Board certification in a relevant field or postgraduate residency with direct patient care.
• At least 1 year of clinical services under a collaborative practice.
• 3+ years clinical pharmacist experience or PGY2.
• Strong communication, organization, and technical skills.
• Demonstrated empathy, integrity, and a team-first mindset.
