The Associate Medical Director will provide comprehensive assessment, planning, and primary care, ensuring continuous medical coverage for participants. This role requires collaboration with community-based physicians and the interdisciplinary team (IDT) in discharge planning, effective communication regarding changes, and coordination of 24-hour care delivery.

Key Responsibilities
  • Provide assessment and planning for participants' care.
  • Collaborate with community-based physicians and IDT in discharge planning.
  • Communicate changes effectively and coordinate care delivery.
  • Maintain care of participants in nursing homes with regular visits.
  • Participate in a rotating night and weekend call schedule.
Required Skills & Qualifications
  • Residency program accredited by ACGME.
  • Master’s degree or doctorate degree.
  • Board Certification in Internal Medicine or Family Practice.
  • Board Certification in Geriatrics preferred.
  • Active California Physician License without restrictions.
  • Active CDS Certification and DEA.
  • Enrolled or eligible to provide Medicare/Medi-Cal services.
  • CPR and First Aid certification within 180 days of hire.
  • Ability to maintain work/life balance with benefits including 17 days PTO, 12 holidays, and sick time.
