Job Overview
As a Care Manager, you will assess, coordinate, and monitor care for members with high behavioral health acuity, collaborating with families, care teams, and community resources to ensure optimal outcomes. You will play a vital role in developing individualized care plans, conducting outreach, and providing education to members and their support networks.
Key Responsibilities
- Develop, assess, and facilitate complex care management activities for mental and behavioral health members.
- Evaluate member needs via phone, digital, in-home, or site visits.
- Design and implement personalized care and service plans for best outcomes.
- Coordinate care among members, families, providers, and community resources.
- Monitor care plans and member status for changes in treatment or condition.
- Educate members and families on procedures, provider instructions, and healthcare benefits.
- Perform additional duties as assigned to support member health and well-being.
Required Skills & Qualifications
- Master’s degree in Behavioral Health, Social Work, or degree from an accredited School of Nursing.
- 2–4 years of related experience in care management or behavioral health.
- Active licensure as LCSW, LMSW, LMFT, LMHC, LPC, or RN with behavioral health experience (state dependent).
- Strong communication, assessment, and organizational skills.
- Commitment to ethical governance, community partnership, and high-quality care delivery.
- Ability to work flexibly in remote, hybrid, field, or office settings.
