Job Overview

Provide comprehensive, community-based primary care for vulnerable seniors within the PACE program. Build long-term relationships with a smaller patient panel, delivering holistic and value-based care.

Responsibilities
  • Perform in-person assessments with the interdisciplinary team to plan care
  • Coordinate discharge and placement for acute and long-term needs
  • Visit about 8 patients per day with home visits
Requirements & Qualifications
  • ACGME-accredited residency required
  • Master’s or Doctorate degree
  • Board Certification preferred
  • Active, unrestricted state medical license
  • CDS certification and DEA registration
  • CPR and First Aid certification within 180 days; hire-ready CPR/First Aid preferred
