Community Health Worker

Job Overview

As a Community Health Worker, you will be responsible for completing initial assessments with patients and families to identify their needs. Building trust through ongoing rapport, empowering patients via motivational interviewing and goal setting, coordinating with interdisciplinary care teams, families, and providers, advocating for patients, and assisting with social services setup are key aspects of this role. Understanding social determinants of health and collaborating with Behavioral Health Clinicians as needed are also crucial.

Responsibilities

• Complete initial assessments with patient/family
• Build trust through ongoing rapport with patients
• Empower patients via motivational interviewing and goal setting
• Coordinate with interdisciplinary care teams, families, and providers
• Advocate for patients to ensure needs are addressed by the team
• Assist with social services setup (food, housing, benefits, etc.)
• Understand social determinants of health (SDOH) and their impact
• Collaborate with Behavioral Health Clinicians as needed for patient care

Requirements & Qualifications

• Bachelor's degree in health-related field or equivalent (preferred)
• Experience with populations with complex medical/behavioral needs
• Experience in community health or public health
• Knowledge of Medicaid population
• Experience documenting in an electronic health record
