As an Outpatient Physical Therapist for Senior Living Visits, you will make outpatient visits to clients in designated territories, perform tests for mobility, ROM, strength, balance, and safety, develop and implement individualized care plans, revise care plans with nursing in the multidisciplinary team, educate clients and families in rehabilitative care, and document observations and interventions in EMR on a touch-screen tablet.

Key Responsibilities

Make outpatient visits to clients in designated territories
Perform tests for mobility, ROM, strength, balance, and safety
Develop and implement individualized care plans
Revise care plans with nursing in the multidisciplinary team
Educate clients and families in rehabilitative care
Document observations and interventions in EMR on a touch-screen tablet

Requirements & Qualifications

Current Pennsylvania Physical Therapist license
Graduate from an APTA-approved program
Ability to work independently and manage time
Strong interpersonal, organizational, and problem-solving skills
Solid computer skills; EMR experience preferred
