Job Overview

As a Social Worker, you will be responsible for conducting psychosocial assessments, developing care plans, and collaborating with an interdisciplinary team to ensure the well-being of program participants.

Responsibilities
  • Conduct psychosocial assessments
  • Develop participant care plans
  • Establish deep relationships with participants
  • Help participants age safely and independently in their homes
  • Lead and participate in care conferences
Requirements & Qualifications
  • MSW required (Master’s degree in social work)
  • Minimum 1 year experience with frail or elderly population
  • Experience coordinating and facilitating care conferences
  • Experience assisting with behavioral health or substance abuse issues
  • CPR and First Aid certification within 180 days of hire
