As a Patient Advocate, you will serve as the primary point of contact for assigned Medicare patients, building trusting relationships and supporting them in understanding care plans and available resources. Your role involves conducting assessments, performing risk assessments, developing individualized action plans, coordinating appointments, and referring to community-based resources.
You will document patient interactions, communicate regularly with supervising clinicians, and demonstrate competencies in patient communication, relationship-building, and advocacy. This remote position requires 3+ years of experience in patient advocacy or care coordination, with a strong understanding of the U.S. healthcare system and excellent communication skills.
